Here, we will discuss quality improvement tools used in quality improvement projects, opportunities for public health and healthcare to collaborate to improve the quality of services, and participate in a dialogue about quality improvement projects between public health and healthcare.

The Michael J. Perlin Student Award winner is Avery H. LaChance

Avery LaChange is a MD/MPH candidate in her final year at the University of Connecticut School of Medicine. Throughout her graduate school training, she has been active in a number of public health initiatives including taking on leadership roles in UConn's Urban Service Track, writing and advocating on behalf of indoor tanning health policy in CT and conducting research on self image, skin tone perceptions, and skin tone altering activities in the CT adolescent population. After graduation, she hopes to pursue a residency in dermatology and is planning to dedicated her career to dermatology as it intersects with public health.

The Charles G. Huntington III Award is Louise-Marie Dembry, M.D., M.S., M.B.A.

Dr. Louise-Marie Dembry has worked as a clinician, teacher, researcher and healthcare epidemiologist in Connecticut for over 20 years. As the director of the Healthcare Epidemiology course at the Yale School of Public Health she has had the pleasure of teaching and working with many MPH students on various projects related to public health and healthcare epidemiology. Dr. Dembry’s career is dedicated to improving patient safety, being a state and national advocate for infection prevention, and being involved in training the next generation of leaders in the field.

The Ira V. Hiscock Award winner is Ann Greene.

Ann Greene is a writer whose poetry and fiction has been published in various journals and anthologized. As a librettist, her operas and music theatre works have been performed in the US and Europe. For 16 years she taught poetry, fiction, non-fiction and memoir writing, storyboarding, test-taking skills, and performance to K-12 and adult students for public schools, colleges, public libraries and various arts organizations. Since moving to New Haven, she has served on the West River Neighborhood Services Corp. Board and is currently serving on the Yale School of Public Health Community Alliance for Research and Engagement (CARE) Steering Committee and the New Haven Urban Design League Board. She is studying towards an M.S. in Secondary Mathematics Education and she is currently an Associate Producer for Jazz, Cancer, and Life: Fred Ho's Last Year.

The C.E.-A. Winslow Award winner is William G Faraclas, Dr.P.H.

Professor William G. Faraclas, is founder of the Public Health program at Southern Connecticut State University, where he chaired the Department of Public Health for 33 years. Dr. Faraclas is Director of SCSU's International Field Studies in Health Program, through which he has taken 26 groups of students to Guatemala and Mexico, and Director of the Environmental Health Training Program, a workforce development collaborative with the Connecticut Department of Public Health. He is recipient of the J. Philip Smith Outstanding Teaching Award (2004) and the Robert E. Jirsa Service Award (2013)-two of the University's top honors. He has been a member of the CPHA Board of Directors since 1985. (He is married to Kara Faraclas and has four children and two grandchildren.)

Charles G. Huntington III Award: The Charles G. Huntington III Award is presented annually to a Connecticut health care practitioner who has demonstrated public health leadership and a commitment to the health and well-being of populations.

Michael J. Perlin Student Award: The Michael J. Perlin Student Award recognizes a current graduate or undergraduate student enrolled in an accredited public health program who demonstrates, through student activities and/or community service, a commitment to leadership, volunteerism, and the highest educational standards.

The awards will be presented at the CPHA Awards Breakfast, scheduled for Thursday, April 10, 2014 during National Public Health Week.
