	<div align="left">Dr. David Jacobs is the Director of Research at the National Center for Healthy Housing in the US.  He previously worked at the U.S. Department of Housing and Urban Development as Director of the Office of Healthy Homes and Lead Hazard Control, where he was responsible for policy development, grants management, enforcement, public education and training, and research. He wrote the first federal interagency strategy on childhood lead poisoning prevention in the US. He also conceived and won Congressional support for the U.S. Healthy Homes initiative in 1999. He has testified before Congress and other legislative bodies on many occasions and has numerous scientific publications. Dr. Jacobs is also an adjunct associate professor at the School of Public Health at the University of Illinois at Chicago, a faculty associate at the Johns Hopkins University Bloomberg School of Public Health, and Director of the World Health Collaborating Center on Healthy Homes Research and Training.</div>

<p>Learn about the implementation of community assessment and improvement plans in our state.</p>

<p><strong>Promising Clinical Preventive Services &amp; New Approaches to Promote the Community's Health</strong></p>
<p>We will identify two community clinical preventative services offered by local health departments/districts, describe outcomes from the local health clinical preventative services, and illustrate the effectiveness of local health interventions promoting community’s health. </p>

<p><strong>Promoting Healthier Choices and Health Careers through Public Health 101: A Workshop for Public Health Mentors and Educators</strong></p>

<p><strong>The Michael J. Perlin Student Award winner is Avery H. LaChance</strong></p>
<p><img style="height: 100px; float: left;" src="https://cpha.site-ym.com/resource/resmgr/Awards_2014/Lachance.jpg">Avery LaChange is a MD/MPH candidate in her final year at the University of Connecticut School of Medicine. Throughout her  graduate school training, she has been active in a number of public health initiatives including taking on leadership roles in UConn's Urban Service Track, writing and advocating on behalf of indoor tanning health policy in CT and conducting research on self image, skin tone perceptions, and skin tone altering activities in the CT adolescent population. After graduation, she  hopes to pursue a residency in dermatology and&nbsp;is planning to dedicated&nbsp;her career to dermatology as it intersects with public health.</p>]]></description>

As the director of the Healthcare Epidemiology course at the Yale School of Public Health she has had the pleasure of teaching and working with many MPH students on various projects related to public health and healthcare epidemiology.  Dr. Dembry’s career is dedicated to improving patient safety, being a state and national advocate for infection prevention, and being involved in training the next generation of leaders in the field.</p>]]></description>

<p><strong>The Ira V. Hiscock Award winner is Ann Greene.</strong></p>

<p><img style="height: 100px; float: left;" src="https://cpha.site-ym.com/resource/resmgr/Awards_2014/Green.jpg">Ann Greene is a writer whose poetry and fiction has been published in various journals and anthologized. As a librettist, her operas and music theatre works have been performed in the US and Europe. For 16 years she taught poetry, fiction, non-fiction and memoir writing, storyboarding, test-taking skills, and performance to K-12 and adult students for public schools, colleges, public libraries and various arts organizations. Since moving to New Haven, she has served on the West River Neighborhood Services Corp. Board and is currently serving on the Yale School of Public Health Community Alliance for Research and Engagement (CARE) Steering Committee and the New Haven Urban Design League Board. She is studying towards an M.S. in Secondary Mathematics Education and she is currently an Associate Producer for <u><a href="http://www.indiegogo.com/projects/jazz-cancer-and-life-fred-ho-s-last-year" target="_blank"><font color="#548dd4">Jazz, Cancer, and Life: Fred Ho's Last Year</font></a></u>.</p>]]></description>

<p><strong>The C.E.-A. Winslow Award winner is William G Faraclas, Dr.P.H.</strong></p><strong></strong>
<p><img style="width: 71px; height: 100px; float: left;" src="https://cpha.site-ym.com/resource/resmgr/Awards_2014/Faraclas.jpg"><a href="http://www.cpha.info/?page=NPHW" target="_blank"><u><font color="#548dd4">Professor William G. Faraclas</font></u></a>, is founder of the Public Health program at Southern Connecticut State University, where he chaired the Department of Public Health for 33 years.  Dr. Faraclas is Director of SCSU's International Field Studies in Health Program, through which he has taken 26 groups of students to Guatemala and Mexico, and Director of the Environmental Health Training Program, a workforce development collaborative with the Connecticut Department of Public Health.  He is recipient of the J. Philip Smith Outstanding Teaching Award (2004) and the Robert E. Jirsa Service Award (2013)-two of the University's top honors.  He has been a member of the CPHA Board of Directors since 1985. (He is married to Kara Faraclas and has four children and two grandchildren.)</p><hr>
