Staff from NACCHO and three local health departments -Whatcom County, WA, Humboldt County, CA, and Madison County, NY -will discuss how to address social determinants of health (SDOH) in rural communities through community health assessment and community health improvement planning (CHA/CHIP) processes. Participants will learn about the Healthy People 2020 Social Determinants of Health framework and discover tools and resources to facilitate SDOH discussions. Participants will also hear practical examples from the three local health departments on their efforts to address SDOH priorities through the CHA/CHIP. This webinar is provided with support from Healthy People 2020/The Office of Disease Prevention and Health Promotion, and the Centers for Disease Control and Prevention.
